Dry needling is not acupuncture, a practice based on traditional Chinese medicine and performed by acupuncturists. Dry needling is a part of modern Western medicine principles, and supported by research. What is a Trigger Point? A trigger point is a taut band of skeletal muscle located within a larger muscle group. Trigger points can be tender to the touch, and touching a trigger point may cause pain to other parts of the body. What Kind of Needles Are Used? Dry needling involves a thin filiform needle that penetrates the skin and stimulates underlying myofascial trigger points and muscular and connective tissues. The needle allows a physiotherapist to target tissues that are not manually palpable. Why Dry Needling? When our physiotherapists use dry needling, it is typically one technique that's part of a larger treatment plan. We use dry needling with the goal of releasing or inactivating trigger points to relieve pain or improve range of motion. It is constantly evolving to keep up to date with the latest evidence and best clinical practice internationally. The effect of ultrasound via an increase in local blood flow can be used to help reduce local swelling and chronic inflammation, and, according to some studies, promote bone fracture healing. The intensity or power density of the ultrasound can be adjusted depending on the desired effect. A greater power density (measured in watt/cm2) is often used in cases where scar tissue breakdown is the goal. Ultrasound can also be used to achieve phonophoresis. This is a non-invasive way of administering medications to tissues below the skin; perfect for patients who are uncomfortable with injections. With this technique, the ultrasonic energy forces the medication through the skin. Cortisone, used to reduce inflammation, is one of the more commonly used substances delivered in this way. As clinicians, it has been our experience (and is well documented) that post injury, patients often develop movement and postural dysfunctions that need to be addressed prior to returning to mainstream exercise activity. The skill in treating headaches starts with an accurate diagnosis. Broadly speaking, headaches fit into the following categories: Migraines: Totally debilitating and accompanied by aura such as perceived flashing lights, blurred vision and light sensitivity. Migraines are essentially a vascular issue and with the correct medication can be very effectively managed. Certain stimuli such as caffeine or hormonal fluctuations may also contribute to the onset of a migraine episode. Tension-type headaches: Caused by excessive muscle tension of the neck and shoulder muscles. TMJ headaches: Headaches caused by excessive spasm or pain originating from the jaw-joint (tempero-mandibular joint). Headaches related to pathology of the upper neck vertebra and the corresponding upper neck joints – for example after a whiplash accident, or with osteo-arthritic changes of the spine. This is a very broad term and is often poorly understood. In order to shed some light on the subject, I thought I would share a true patient presentation and treatment with you. Let's call my patient 'Peter'. He is a high achieving investment banker who has had been quite stressed of late. He exercises regularly, particularly enjoying trail runs and cycles. He presented at the practice complaining of pain and numbness in his left index finger. He could not remember hurting his finger and so could not understand why he was experiencing these disturbing symptoms. After a thorough examination, I diagnosed Peter with referred pain originating from his an irritated nerve root in his neck. Peter was somewhat skeptical initially, however, after mobilising his lower neck vertebrae and after releasing the tight muscle structures around his neck, he was amazed that the pain and numbness in his finger had completely resolved (after 30 minutes of treatment). However, he now felt a dull ache in the area of his left shoulder blade. I explained that this was actually an excellent sign as it meant his pain was 'centralising', or 'becoming more localised' - an indication that we were on the right track! How does this work?
